In light of recent developments related to our community response to COVID-19, I would ask that you read the following screening questions before booking or coming in for your appointment.

Have you had close contact with anyone with acute respiratory illness or travelled outside of Ontario in the past 14 days?
Do you have a confirmed case of COVID-19 or had close contact with a confirmed case of COVID -19?
Do you have any of the following symptoms?

• Fever
• New onset of cough
• Worsening chronic cough
• Shortness of breath
• Difficulty breathing
• Sore throat
• Difficulty swallowing
• Decrease or loss of sense of taste or smell
• Chills
• Headaches
• Unexplained fatigue/ malaise/ muscle aches
• Nausea/ vomiting/ diarrhea/ abdominal pain
• Pink eye (conjunctivitis)
• Runny nose/ nasal congestion without other known cause

 
When you arrive at the clinic, please observe the following:

• Wear a face mask
• Arrive no more than 5 minutes before your appointment
• Wash/sanitize your hands upon arrival
• Family members should not accompany you into the clinic, unless essential.
